How to Become a Patient
The Center welcomes you, we want you to help you have a good quality of life. But first we need you to answer some questions:
Do you have a referral letter from your doctor?
Did you fill in the information in our new patient information kit?
So that the time you have with the doctor is spent more effectively at your visit, please download our New patient information kit which has information to help us and yourself before your first visit.
The kit asks for information such as:
- How long have had the pain?
- Were you treated by another pain center?
- Have you been to us in the past?
- What type of insurance do you have?
Once you have completed New patient information kit you can mail, fax or drop it into our office before your appointment. You may also be required to sign our opioid agreement.
